Brevard Family Partnership’s System of Care is family centered, strength-based and community driven. We believe that all children have the inalienable right to grow up safe, healthy and fulfilled in families that love and nurture them.
Our System of Care facilitates a single point-of-entry model. It is this model that brings consistency to the children and families as they enter the system, and provides the team with a standard assessment of the child. This assessment enables the care manager to meet the needs of the child upon entry into the system to ensure proper initial placement of the child.
It is imperative that the first placement of a child is appropriate, as he/she needs stability and security in order to manage this crisis in his or her life, with minimal emotional damage.
Our System of Care includes the following services to most effectively meet the needs of the children and families we serve:
Mobile Response: Teams are mobilized to assist in crises and stabilize the family in need, if appropriate. This is a 24/7 service that helps ensure the safety of the children and the stability of the homes in which the child resides.
Family Team Conferencing: Comprised of family members, friends, care manager and others in the community who may have a relationship with the family (e.g. teachers, clergy, etc.), this team is created to provide the family with the necessary support to ensure the success of their care plan. It also provides the family with a voice and ownership in their plan. Data indicates that this element is a pivotal factor to the success of the families utilizing our services.
Flexible Supports: Plans adjust to the needs of the family. As one plan does not fit all families, one plan does not fit one family at all times. Many families experience different crisis at different times in their life, which would lead to a variety of services.
Wraparound: We build a network of support around a family. The wraparound philosophy is one that ensures the family’s needs are met with the appropriate services at the appropriate time.
